IRB - Application Forms
Photocopies of the general IRB Application Form are available at
the Office of Institutional Research.
You may also download the IRB Application Form as a Word
document.
Other IRB Forms:
1) Application for Extension of IRB Approval: This
form is to be completed by the researcher (and his/her advisor, if
appropriate) in order to receive an extension if the project is
delayed beyond the dates specified in the original proposal approved
by the IRB.
2) Brief Application for Continuation of Classroom Research
Projects: A form for Blackburn faculty and staff—for
expedited or exempt reviews only.
3) Sample Consent Form: Use this form as a template
to prepare your consent form.
4) Sample Assent Form: Use this form as a template
to prepare your assent form.
Special Note: A helpful strategy for filling out the Word
documents is to double-click on the underlined area that highlights
the underlined area, and then start typing. This deletes the
underline and replaces it with your typed response. Similarly, where
large open areas are provided for a response, you should delete the
blank lines in order to use the space taken up by the blank carriage
returns for your response. If items move substantially or become
misaligned as a result of your deletion, you may have to add spaces
or carriage returns in order to move the affected part back. In
general, please fill out the form in such a way as to preserve
spacing and pagination so the IRB readers can easily follow your
application text.
